Healthcare and Insurance

• Comprehensive health insurance through a choice of either a Point of Service (POS) plan or a HMO plan. Coverage is effective first day of month following thirty (30) days of employment which include prescription drug benefits and vision reimbursement program
• Comprehensive dental insurance. Coverage is effective first day of month following thirty (30) days of employment
• Flexible Benefits Account, which allows for pre-tax contributions to a Healthcare Account and/or Dependent Care Account
• Company-paid life and AD&D insurance coverage (1X salary), effective ffirst day of month following thirty (30) days of employment. Additional voluntary life and AD&D insurance coverage is available for you and dependents at a discounted rate.
• Short term disability coverage 50% Company-paid 50% employee paid effective ffirst day of month following thirty (30) days of employment.
• Company-paid short- and long-term disability coverage, effective ffirst day of month following thirty (30) days of employment.
• Company paid Employee Assistance Program (EAP) eligibility first day of month following thirty (30) days of employment for employees and their family for free short-term counseling, referral services for legal, financial, elder care, child care and work-life resources.

401(k)

• 401(k) Program with employer match. Eligibility for employee contributions begins on the first of employment. Employees may contribute up to 60% of salary (or the applicable IRS maximum); employer matches 100% up to the first 4% of employee contributions beginning immediately upon participation in the plan.

Holidays and Paid Time Off

• Seven paid holidays per year plus three floating hoidays per year.

• 18 Paid Time Off (PTO) days each year of first three years employment and one additional day for each year through eight year employment Employees with the Company for 10 years or more accrue a total of 25 days per year.

• Company will cover cost of initial licensing education, testing, first test re-take (if necessary), state licensing fees, license renewal fees and required continuing education credit hours for employees working 30+ hrs/wk
